The Hourglass Tummy Tuck™ and All-Core Tightening

tummy tuck diagram

In addition to removing excess fat and loose skin, abdominal muscle separation (diastasis recti) is repaired. The technique pulls in the side oblique muscles, too, bringing together the left and right side of the rectus abdominus and transverse abdominis for total muscle tightening and a cinched-in waist. Plus, there’s no puckering of the skin from this plastic surgery procedure, so you’ll have a smoother, firmer contour!

Why the Hourglass Tummy Tuck™ is Better

1. You get a tightening of the underlying abdominal muscles and obliques to bring in your waist, creating an hourglass figure. It’s like you get an internal corset that’s super long lasting. Dr. Kesarwani is one of the few tummy tuck surgeons who tightens not only the rectus muscles that get spread apart with pregnancies, but also the oblique muscles to re-establish the waistline.

2. Dr. Kesarwani is very meticulous when it comes to incisions and stitching them up. This means that you’ll have symmetric incision lines that are straight, even and neat. This gives you tidier scars that remain covered up by underwear or bikini bottoms.

3. Dr. Kesarwani applies advanced surgical techniques that reduce trauma and bleeding. You’ll have less bruising and swelling than other methods of tummy tucking, so you have a faster recovery.

Results may vary.

Healthy people who have realistic goals and follow a healthy diet and exercise routine are good candidates. Abdominoplasty is a popular surgical procedure that’s commonly part of a mommy makeover or following weight loss.

You should be at your ideal weight to reduce surgical risks (such as wound healing complications and deep venous thrombosis). This also improves outcome predictability and allows results to last longer. If you aren’t at your ideal weight, please speak with Dr. Kesarwani about your situation.

Mini Tummy Tuck or Full Tummy Tuck?

The easiest way to figure out whether you need a mini or full is first pinpointing the abdominal area(s) that you want to improve.

What is a mini tummy tuck?

A mini tummy tuck flattens the lower mid-section. It includes fatty tissue removal, loose skin tightening and repair of lax abdominal muscles but only below your belly button (lower abdomen). Anything above is left unaffected. Generally speaking, a mini tummy tuck achieves limited improvement because the area of focus is smaller.

What is a full tummy tuck?

The most common option, a full improves everything below and above your belly button. It’s as simple as that! Tummy skin tightening, fatty tissue removal and muscle repair of the abdominal wall is targeted, giving you a more dramatic improvement than a mini tummy tuck. Liposuction can be performed at an additional cost for further enhancement of harder to reach areas and to refine the waist contour. This option is commonly performed on most patients who’ve had significant weight loss or had children. Please book your consultation in order to find out the price of your tummy tuck and discuss factors that would contribute to your surgery costs.

Preparing for Your Tummy Tuck

Dr. Kesarwani believes that the difference with his approach is all in the measuring, marking and planning of your plastic surgery to acchieve natural looking results. This sets your expectations and allows you to know what to expect. Dr. Kesarwani will make markings while you’re sitting and standing. These are positions that you would see yourself doing on a regular basis after surgery. That way you’ll look great, whether you’re standing straight up, bending over or lying down.

The Procedure — How is a tummy tuck performed?

Dr. Kesarwani administers a unique formulation of anesthesia. This decreases any discomfort during your surgery and for about 6-8 hours afterward. We place pumps on your legs to prevent the risk of blood clots and deep venous thrombosis.

The skin is lifted off the abdominal wall so the upper flap can come down. Loosening the tissues to a certain extent also allows for a lower incision line. Dr. Kesarwani leaves the skin on your sides, so it remains attached. Doing this maintains blood flow, minimizing the risk of complications (i.e. necrosis). He also uses electrocautery to reduce bleeding and bruising. Excess fat is removed (with liposuction, if necessary). There’s only a conservative amount of skin removed to avoid a tight, unnatural result.

When it comes to abdominal wall muscle tightening, though, Dr. Kesarwani is not conservative.

He doesn’t only repair diastasis recti down the mid-line of your body. He also pulls in the oblique muscles. Targeting the muscles underneath gives you a more cinched-in waist for a smaller girth that resembles an hourglass. It’s kind of like an internal corset, but instead of tying it up in the back, it’s done in the front without visible seams. The "internal corset" remains tightly together by sutures. It’s also supported by the fatty tissues that stick to these muscles, holding it in place like glue.

Dr. Kesarwani takes a fair amount of time to ensure the incision around the navel isn’t all the way around. He does this by repositioning the hanging skin on the lower abdomen up into the navel, where the incision remains below the surface of the skin. Because it’s your own skin going inside the navel, there is no visible scar. This attention to detail allows you to flaunt your midriff without worrying about showing a scar!

Dr. Kesarwani then closes the incisions after removing excess skin. He administers local anesthetic in the abdominal area. This decreases discomfort for several hours after your cosmetic surgery. Pain medication is prescribed.

Dr. Kesarwani places sutures in several layers of tissue, so that there is less tension on the skin. This gives you a smoother, finer scar that’s less at risk of re-opening. There is no lumpiness. Drainage tubes are placed within in the incision to reduce fluid accumulation.

The entire procedure can take 1 hour or more, depending on what is done. While the general anesthesia adminstered by your anesthesiologist wears off in a couple of hours, the local anesthesia medications focused on the treated area provides longer lasting relief.

Tummy tuck drains help to get rid of any fluid buildup that occurs within the cavities of the treated area.

Drainless vs. Drain Tummy Tuck

There are several surgeons who don’t place drains at the end of the tummy tuck surgery in Ontario (known as a drain free tummy tuck). Dr. Kesarwani uses drains for the following reasons:

A drainless tummy tuck involves sutures to decrease cavities that may fill with fluid. This can lead to puckering on the skin surface that either took too long to go away, or didn’t go away at all. Puckering and unevenness can lead to a poor aesthetic outcome following a drain free tummy tuck. These issues are common drainless tummy tuck complications. Using drains eliminates fluids, thereby reducing swelling and speeding up recovery.

Using drains causes some suction. The underside surface of the fat sticks on to the upper surface of the muscles. This supports muscle tightening and kickstarts the healing process, so recovery is shorter. The suction also helps puckering go away faster, too.

Drain marks are often found in the pubic area but you won’t have any. This is because Dr. Kesarwani places drains in the main incision. There needs to be no extra incisions made.

Once the surgery is over, you’ll stay in our recovery room for a few hours. Our dedicated nurses and Dr. Kesarwani will observe you until you’re ready to go home. A family member or good friend should help you get home.

Wearing Your Compression Garment During Tummy Tuck Recovery

During recovery from abdominoplasty, we recommend tummy tuck patients wear a compression garment, or abdominal binder to:

  • Support the repaired tissue.
  • Minimize swelling and bruising, particularly in the lower abdomen.
  • Help loose skin retract and form to its new shape.
  • Control scar formation.
  • Lead to smoother skin and better results.
  • Decrease pain.

Compression garments are generally worn for four to six weeks. You may need to wear the garment 24/7 for the first week, and then ten to twelve hours a day afterward. After this time, you can still wear the compression garment products for comfort or pain relief if you’d like.

Since each patient is different, the best thing to do is to ask us directly how long you should wear a compression garment.

Full recovery after this elective cosmetic surgery usually lasts 3-6 weeks. During this time we will advise you on what kind of activity to avoid, how long to rest, and how to treat the area. It is usually advised that abdominoplasty patients take some time off of work, avoid physical activity, especially heavy lifting and stop smoking.

A Note on Scars

We have found that CytoDerma™ is an essential part of post-treatment recovery to help scars fade as much as possible. This silicone gel should be applied for three months.

Read more about silicone scar gel here.

Surgical scars tan like the rest of our skin when exposed to the sun. However, they often do not fade in colour like the rest of our skin. You don’t want dark scars, so diligent sunscreen use is particularly important for the first 12-18 months after surgery.

Are Tummy Tuck Results Permanent?

The results are permanent but can be affected by weight gain, aging and future pregnancies. Follow a healthy lifestyle and remain at a stable weight to maintain your results.

It’ll be amazing how much of a positive impact the surgery will have on your confidence. You’ll look like you did thousands of situps for a leaner, firmer mid-section and less stretch marks! Our abdominoplasty patients love their new hourglass figure, and so will you.

How Soon After Childbirth Can You Have A Tummy Tuck?

The importance of timing concerns cannot be understated when considering a post-pregnancy tummy tuck. The average post-pregnancy body is not immediately in ideal condition for surgery. Most surgeons recommend that you wait 6 months or so after giving birth before you get a tummy tuck.

Breast Feeding will be an Important Consideration

Women often breastfeed their babies for the first few months of their baby’s life. During this time, it’s unadvisable to undergo a tummy tuck due to the short-term impact the surgery will have on the patient’s body. This includes other surgical procedures as well, such as a breast reduction and cosmetic breast surgery. Women should wait at least six months after stopping breast feeding before they approach the tummy tuck procedure.

Other important considerations include the following:

Post-Operative Care
Oftentimes, tummy tuck patients require at least 2 weeks recovery after surgery. This can limit mothers in terms of the activities they can complete with their children during that time. There are also restrictions on physical activity for a specific length of time, which includes lifting and carrying children.

Long-Term Tummy Tuck Benefits
For many women, the long-term benefits of the procedure must also be considered. For example, they may feel a heightened sense of confidence about their appearance. This may help enhance their lifestyle.

Is Tummy Tuck Surgery Safe After Any Stomach Surgery?

If you’ve had previous surgery targeting your stomach, you may still move forward with a tummy tuck abdominoplasty. However, it’s important to consider the following for this body contouring procedure:

Location of Scar a Critical Consideration

Depending on the location of the previous surgery, such as hernia repair, most can be completed on patients who have undergone a stomach operation. However, our surgeon may recommend that the patient undergo a variation on the tummy tuck procedure as a result of their previous surgeries. Depending on the degree of scar from the past surgery, it may conflict with the circulation of abdominal skin during a tummy tuck, making the surgery more challenging. In most cases, when the scar is low enough on the body, it can be removed along with excess skin during a tummy tuck. But our surgeon will have the final say on whether the procedure moves forward.

The Following Patients Remain Suitable Candidates

Some patients may have been put off because they have been misinformed that their past surgeries would prevent the process being completed safely. But with the right surgeon, these tummy tuck patients can safely achieve the toned stomach and ideal figure they want if they’re at a stable weight:

Patients who Have Undergone C-Sections
Mothers who have undergone C-sections are often prime candidates for the tummy tuck procedure. The patient will be able to remove the excess fat around their stomach and may even be able to remove some scar tissue depending on the location on the body of the C-section and the extent to which the body has recovered from the process.

Patient Who Have Undergone Appendectomies
The appendectomy procedure is relatively common across the country, and this means a large section of the population might falsely believe they are unable to undergo a tummy tuck due to their appendectomy scarring. Fortunately, with the innovation in surgical procedures in recent years, appendectomy scars tend to be quite small and simple for skilled surgeons to work around.

Patients Who Have Undergone Tubal Ligation
Women who have undergone tubal ligation are also prime candidates. The scars from the process can be removed by a specialist when completing the tummy tuck.

Of course! For a natural result, your navel is preserved but the shape may improve. It may go from a flattened out oval to a rounder shape.

What happens to the belly button during a tummy tuck abdominoplasty?

Belly buttons are actually the remnants of our first scar because they form after the umbilical cord is cut upon our birth. When we undergo an abdominoplasty, our belly buttons can appear as though they have been damaged or scarred. However, this is all part of the healing process. The navel is attached to a stalk, and it remains attached during the surgery. During full abdominoplasty, a new opening is created in the skin to which the navel and stalk are attached. These are untouched during mini abdominoplasty. Depending on where your navel sits on your abdomen, it may be shifted to accommodate the new, tighter contours of your abdomen.

For full abdominoplasty, swelling and crusting over of the scabs can take a few months to resolve. There may be some swelling in the first few days after your procedure. Scarring is normal and the scar itself will be hard at first but soften over time. It can take anywhere from three weeks to six months for your navel to fully heal. Some crusting around the scar site is normal, and this will also clear up after about three weeks. Most belly buttons recover nicely. However, if you notice anything out of the ordinary, it could be cause for concern. Look out for:

– A navel opening that appears to constrict or contract
– A scar that gets thicker instead of fading over time
– A navel that looks distorted, elongated, or oddly-shaped
– Formation of pus or blood or other substances

If you notice that after six months your navel hasn’t healed and remains red and crusty, contact us immediately. It could be an infection that requires antibiotics.
